Problem / Objectives |
Primary liver tumors in children and adolescents are rare and affect 0.3 to 1% of all paediatric tumors. In Central Europe, the incidence is 0.5 to 2 cases per 1 million children. The most common hepatocellular malignancies are hepatoblastoma (HB) in infants and young children and hepatocellular carcinoma (HCC) in schoolchildren and adolescents. The malignant, non-epithelial tumors from the group of sarcomas, but also germ cell tumors, rhabdoid tumors and others are much rarer.

Hepatoblastoma
Hepatoblastom (HB)-Patienten werden für eine bessere Therapiestratifizierung in eine Standardrisikogruppe (SR-HB) mit nicht metastasiertem, potentiell resektablem HB und eine Hochrisikogruppe (HR-HB) mit Tumorbefall der gesamten Leber und/oder
Hepatozelluläres Karzinom
Therapieempfehlung für Patienten mit Hepatozellulärem Karzinom (HCC): Für das kindliche HCC wird, wann immer möglich, eine primäre Resektion empfohlen. Als medikamentöse Therapie wird eine Therapie mit Cisplatin und Doxorubicin (Czauderna et al., 2002) vorgeschlagen, dies je nach individueller Situation ggf. kombiniert mit Sorafenib, für das beim HCC des Erwachsenen (Llovet et al., 200 8) und bei einzelnen Fällen von HCC bei Kindern und Jugendlichen (Schmid et al., 2009) eine signifikante Wirkung gezeigt wurde. Patienten mit primär nicht resektablen Tumor sollten präoperativ 4 Kurse erhalen, nach Resektion erhalten alle Patienten 2 Kurse. |
